This is our chance to get an idea of what your dental needs are and what, if any, kind of dental treatment you need in the immediate future. As we check for signs of gum disease and tooth decay, we’ll ask about any current issues with your oral health that you might have noticed.

It can be time-consuming and awkward to try and deal with clip boards, insurance cards, and prescriptions on the day of your appointment, so to save time and make things easier on you, we encourage you to print and fill out your new patient forms right here.
